You have the right to a Good Faith Estimate of your health care costs

If you do not have health insurance, or you choose not to use your insurance for a service, federal law generally requires your health care provider to give you a written Good Faith Estimate of expected charges when you request one or schedule care at least three business days in advance. You can ask Lifebulb for an estimate before deciding whether to schedule services. For ongoing therapy or medication-management visits, ask which services and time period the estimate covers.

When you should receive it

If you schedule an appointment 3–9 business days before the service, the estimate generally must be provided within one business day after scheduling. If you schedule at least 10 business days before the service, the estimate generally must be provided within three business days after scheduling. If you request an estimate before scheduling, it generally must be provided within three business days after your request. An estimate describes expected charges for the reasonably anticipated care identified in it. It is not a contract requiring you to receive care. Ask each provider involved in your care about that provider’s expected charges, and contact us if the planned services change.

If your bill is higher than the estimate

If a bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process. You generally must start the dispute within 120 calendar days after receiving the initial bill. Keep a copy of your estimate and your bills.
